This was your potential scenario for (A):

I'm confused why you don't have M assigned somewhere, since you have 7 of the 8.
The problem with the scenario you've written is that G and F are too close to each other.
As you said, there need to be 3 in between them (in both directions).
Only P and K separate them on one side ... this breaks the 1st rule.
If you had GPMKFIOH, you'd be okay, but that obviously would no longer be (A).
Fyi, (B) also breaks this rule because we see neither F nor G. That means they're together in the string of 4 letters leftover. That means they're too close together.
In order to "see" rule 1 being adhered to for Q18, you'd want to verify that an answer choice has EXACTLY 1 of F or G. (That still wouldn't guarantee that there were 3 in between them in each direction, but it would at least be possible.)
