is my diagram incorrect?

Really close. But your diagram doesn't take into account the first constraint. Remember that F is on the same shelf as either V or W. The question stem now asks us to put F and H on the same shelf.

The resulting hypotheticial should look like

1. P/S

2. S/P W

3. V F H
