SC Strategy Guide

The SC guide says that ---
"Sometimes the original sentence will have a clear, unambiguous meaning. In these cases,
your goal is to preserve this original meaning as you correct other issues. Do not alter the
author's intent when you make your choice!"

a) Do we have an example for the same.

b) Also, I am a little confused here as to if the author intends to say something in the answer A so can an answer choice B/C/D/E be ruled out on the basic that one of them gives a different meaning to the sentence.

there is nothing sacred about answer choice A. when we talk about the "original" meaning, we don't mean the meaning in answer choice A; we mean the intended meaning, i.e. the meaning of the correct sentence. you will usually get a good general idea from reading the sentence (with ANY of the answer choices plugged in) what the meaning was supposed to be, and you have to make sure the final answer adheres to this intended meaning. if you had to stick with the meaning of answer choice A, then literally 100% of sentence correction questions would have A as the answer!
