Found this in SC Strategy guide (Pg 150 Q7e) and seeking help. How is the below correct? What would be the meaning of this sentence?

"Many places are called Naples - not only the cities in Italy and in Florida but a town in Clark County, South Dakota.

I'm guessing that you actually know the meaning of this sentence. If you honestly don't and aren't just fooling yourself into believing that you don't understand it, let us know and we'll try to help you out.

As for why the sentence is correct, this is NEVER what you should be asking about sentence correction. Your only job is to find mistakes. While it is appropriate to ask why something is WRONG, it is never relevant or helpful to ask why something is right. Keep this in mind, and you will do much better in sentence correction!

I kind of agree with the op here. Shouldn't we use "but also" instead of "but" in this sentence? Is "not only... But..." a valid construction on the GMAT?

"also" is not strictly necessary.
the point is that "not ... but ..." is a parallel construction. adding adverbs (such as "also") alters the meaning, but doesn't affect whether the construction itself is legitimate.
