This sentence is made by me:

By 1972, ten of the states of country X had granted women the right to vote, but only one in the East.

Which is correct:

1. only one in the East had

2. only one eastern state

Usage of past perfect.Moreover need to maintain parallelism had......had.....

sudhan is correct that only #1 exhibits proper parallelism, but #1 has problems with clarity: i can't really tell what it's supposed to say.
- is 'the East' part of country X?
- the structure strongly suggests that the Eastern state mentioned is separate from the other ten. if it's one of them, then you should say something like
[i]by 1972, ten of the states of country x, only one of which was in the country's eastern region, had granted...
