It's good that you have lots of evidence, both long-standing and recent. It's tough to get accommodations on this test, but it can be done if you have the right paperwork. I have spoken before with students who have gotten accommodations and they have recommended finding an evaluator who has specific experience with GMAC. From what I've heard, it's much tougher to get their approval than it is for SAT, etc, so an evaluator who has written evaluations specifically for the GMAT is more likely to know what to document / say in order to get you approved.
In terms of preparing, until you hear whether you have been approved, study as though you are not going to get approved. Better to be over-prepared than under-prepared!
